Nowadays, it easily ranks as one of the must-watch TV masterpieces of the ’90s. With each episode reaching tens of millions of viewers during the show’s heyday, a massive audience was tuned in when two versions of the decade’s riskiest episode aired on the same night in 1997. ER produced a live episode twice, in just a matter of hours, for two time zones. Even now, nearly 30 years later, live episodes of scripted dramas are rare.

The live episode was the season 4 premiere, called “Ambush,” and aired on September 25, 1997. The first broadcast took place that evening at 10 p.m. ET for viewers on the East Coast, and then three hours later at 10 p.m. PT for the West Coast. “Ambush” didn’t have the most complicated storyline out of the show’s 15 seasons, but the ER team found the perfect format to cover any flubs made by the cast or crew during the live filming.

The story unfolded via the perspective of a documentary crew, filming the County General emergency room in the wake of Dr. Greene’s violent attack in the previous season. Even if not for the live element, “Ambush” would have been a standout episode. In the span of one hour, ER introduced Alex Kingston as Dr. Elizabeth Corday, established that Carter would be starting from scratch after switching from Surgery to Emergency Medicine, nearly killed off Dr. Morgenstern, confirmed that Dr. Greene was (very) slowly but surely recovering, and delivered good news for fans of Doug and Carol’s tumultuous romance.

Considering that the majority of NBC’s live broadcasts at the time more or less consisted of the nightly news reports, sports, and Saturday Night Live, taking this approach to ER was revolutionary at the time, and still impressive in 2026.

                        Why ER Decided To Do A Live Episode

<picture>
        </picture>
Mark and Doug looking at a documentary camera in ER season 4
NBC

ER was already a hit show after three seasons, so it was a safe bet that the medical drama would have a massive audience for the season 4 premiere in 1997. NBC heavily promoted the novelty of going live for the first episode of season 4, which was written by executive producer Carol Flint and directed by Thomas Schlamme. In addition to the series regulars who returned from season 3, “Ambush” credited William H. Macy as special guest star for his return as Dr. Morgenstern. Macy shed some light on why NBC went live for ER back in 1997, telling Television Academy:

It was scary. I only had four or five scenes, but it was live television. It was scary. I think there was a delay in case disaster hit, but it was live television.

Macy credited George Clooney with the idea of going live for an episode, but the finished product was certainly a collaboration between many different people. Interestingly, Clooney didn’t have a particularly large role for “Ambush,” which was mostly carried by Anthony Edwards. The format meant that full rehearsals were necessary, and the cast went through the entire episode a total of eight times by the end of the night on September 25, 1997. Executive producer John Well spoke to Daily News (via Newspapers.com) at the time about the challenges:

The whole process has been more exciting than daunting… The process we’re going through is very similar to staging a play. There are extensive rehearsals and then the performance.

NBC did have a contingency plan, in case there were technical difficulties. One of the rehearsals was taped, and could have aired in case the live signal from Los Angeles was lost during the West Coast broadcast. That presumably meant especially high stakes for the cast during that particular rehearsal, on top of needing to be ready for two live broadcasts in one night, with just a couple of hours in between.

Wells went on to share that “All of what’s going on has to be choreographed,” and the “entire background has to be rehearsed.” Since ER is currently available streaming on both HBO Max and Hulu, subscribers in 2026 won’t have trouble finding “Ambush” to see exactly what Wells meant about rehearsing the entire background.

The episode also went above and beyond to prove to viewers that everything was happening live in primetime. The real-life Major League Baseball game between the Chicago Cubs and Houston Astros was playing on the emergency room’s break room TV, and television production in the mid-’90s didn’t have the kind of technology to easily fake it.

                        Even After Nearly 30 Years, ER’s “Ambush” Episode Is Still Impressive

<picture>
        </picture>
Justina Machado guest-starring in ER season 4
NBC

Television production definitely does use more advanced technology than the ER team had access to in the ’90s, which is apparent enough on The Pitt as Noah Wyle’s second medical drama. That said, “Ambush” is still incredibly impressive nearly three decades after it originally aired on network TV. In fact, not many dramas have dared to try it over the past 30 years. The West Wing, also on NBC, went live for a season 7 episode in 2005, but most scripted shows that have aired live episodes have been comedies.
